In order to tryout and participate in athletics at Resurrection College Prep High School, ALL 4 (four) documents must be completed and turned in BEFORE tryouts:

IHSA Testing Policy & Agreement

IHSA Pre-Participation Exam

RESHS Waiver for Medical Consent

IHSA Concussion Information Sheet
