Medical Forms

Required Medical Forms – Class of 2030

To ensure your daughter can start school safely and participate in all activities, all incoming freshmen must submit the required medical forms by August 1, 2026.

Certificate of Health Examination – Completed by a physician on or after 8/17/2025; includes Health History signed by a parent.
Dental Health Examination – Must be completed between 11/15/2025–5/15/2027.

Allergy, asthma, or diabetes action plans for students carrying medications
Self-Medication forms for inhalers, EpiPens, or other prescriptions
Sports Physicals for students participating in Bandit Athletics (does not replace the Certificate of Health)

Summer School Information

Bandit Summer School 2026

Class of 2030

Bandit Summer School provides tailored opportunities for students to explore creative interests, strengthen foundational skills, and advance toward their 2026-2027 academic goals.

Two times will be offered: Monday through Friday (5 days)- 3 Weeks

Sessions will meet from 7:45 a.m.-12:00 p.m. or 12:30- 4:45 p.m. Each semester class will have one 15-minute break.

Session I: June 8 – June 26, 2026 (no school June 19th- Juneteenth Federal Holiday)

Session II: July 6 – July 24, 2026

To sign up for registration, you must first have your child registered to RES.

To enroll for summer school, fill out this form which will secure a spot for the selected session.

Returning Course Offerings

Health (0.5 credit) — Graduation Requirement
Focuses on personal wellness, mental health awareness, and practical life habits that support long-term success.

Physical Education 1 (0.5 credit)
Emphasizes personal fitness, teamwork, and foundational movement skills through structured physical activity and wellness education.

Physical Education 2 (0.5 credit)
Builds on physical fitness concepts with advanced conditioning, leadership in team activities, and continued development of lifelong healthy habits.

New for Summer 2026

Looking for something innovative or creative? This summer introduces exciting new opportunities:

Creative Writing (Grades 9–12) — 0.5 credit

Explore storytelling, voice, and imagination through short fiction, poetry, personal narrative, and creative nonfiction. Students will:

Develop compelling characters and settings
Craft engaging dialogue
Strengthen descriptive language
Refine editing and revision skills
Express ideas with clarity and confidence

Through guided workshops, peer feedback, and dedicated writing time, students will discover their voice and grow as confident communicators.

Summer Studio: Art & Design Lab (0.5 credit)

A redesigned, hands-on creative studio experience created specifically for the summer term. Students will explore drawing, design, color theory, and creative expression in a project-based workshop environment.

This course will be taught by a different instructor than our academic-year Art teacher and is intentionally designed as a dynamic studio experience tailored for summer learning. It is ideal for students interested in exploring art in a fresh environment or strengthening skills for future electives.

ACT/SAT Prep Class (0.5 credit)

Structured preparation that includes test-taking strategies, timed practice, and skill-building in reading, math, and writing to increase confidence and readiness for standardized testing.

Dance 1 (0.5 credit)

Dance 1 introduces students to foundational movement techniques, rhythm, and choreography across a variety of styles. Students will develop physical coordination, expressive movement, and an appreciation for dance as both an art form and a means of communication.

Design Your Summer with Purpose

Bandit Summer School allows students to:

Lighten next year’s academic load
Open space for electives or advanced coursework
Strengthen college readiness
Stay aligned with graduation requirements

Enrollment will be based on student interest and course viability, so early conversations matter. Families are encouraged to connect with their school counselor to discuss how summer coursework fits into their overall academic plan.

Tuition & Refund Policy: $350 per half−credit course. No refunds will be given once summer school begins.

Attendance Policy: Students are allowed only one (1) absence per session; if a student is absent more than once, she will be dropped from the course without a refund.

Fall athletic tryouts begin before school starts. Please plan summer vacations accordingly.

MEDICAL FORM REQUIRED
A current Certificate of Health Examination or IHSA Sports Physical must be submitted to our Registrar at cwarren@reshs.org at least 48 hours prior to tryouts to ensure eligibility.

At Resurrection College Prep, we are committed to meeting students where they are and helping them grow with confidence. Through our partnership with Superior Chicago Tutoring — also known as Chicago Superior Tutoring — we proudly offer the Title I R.I.S.E. Program — Resurrection Instruction for Student Excellence.

R.I.S.E. is designed to provide eligible students with structured, personalized academic support built right into the school day. By incorporating tutoring into their schedules, students receive consistent, focused instruction that strengthens skills, builds confidence, and supports long-term success.

At Resurrection College Prep, we are dedicated to supporting students with diagnosed learning differences by providing both curricular and executive functioning assistance. Our team of Learning Interventionists works closely with students to enhance skills in reading comprehension, written expression, math, and executive functioning. This specialized support is available both inside and outside the classroom, ensuring students have the guidance they need to succeed academically and develop lifelong learning strategies.

Classroom & Testing Accommodations
We provide accommodations designed to meet individual needs, including Student Accommodation Plans developed from IEPs or 504 Plans.

Academic Accommodation Plan Framework

Resurrection’s Academic Accommodation Plan Framework provides families and students with guidance on how to access the curriculum when a diagnosed disability creates learning barriers. The plan outlines supports, strategies, and accommodations—like extended test time, preferential seating, or academic check-ins—while maintaining college-prep expectations. It also explains eligibility, required documentation, and how accommodations are implemented and monitored.

Mission & Ministry

Campus Ministry is at the heart of our Catholic and Lasallian identity at Resurrection College Prep. We accompany students as we invite them to grow in their relationship with God, explore their faith, and build a supportive community rooted in charity and truth. Throughout the year, students are offered many opportunities for prayer, reflection, service, and faith formation.

As you begin your journey at Resurrection, we want to draw your attention to two important parts of the Campus Ministry program that fulfill graduation requirements.

Service Requirement

At Resurrection College Prep, we believe faith comes alive through service. Students complete a yearly service requirement as part of their spiritual and leadership formation. More information about service expectations and opportunities will be shared with students during the first month of school, and parents will receive details through the school newsletter.

Retreat Requirement

Each student participates in a class retreat every year at Resurrection. Rooted in Catholic theology and Lasallian spirituality, our retreats are designed to be welcoming and meaningful for students of all faith traditions. These experiences are developmentally appropriate, reflection-centered, and thoughtfully designed to support adolescents’ faith development. By the time they graduate, students will have developed tools for meaningful reflection and a deeper understanding of how faith calls us to live day to day.

In addition to these requirements, students are invited to participate in many optional opportunities throughout the year, including prayer experiences, liturgies, service immersion trips, and student programs. We look forward to introducing students to these opportunities as they begin their journey at Resurrection.

Our community is strongest when families are engaged. Parents and guardians are encouraged to consider joining the Women’s Club, Men’s Club, or Athletic Booster Club.

Women's Club

We invite all mothers, aunts, grandmas, and other female figures to join the Resurrection Women's Club at monthly meetings, various school events and activities. There are so many wonderful ways you can be a part of the community of RES and we are dedicated to volunteerism in support of the Resurrection College Prep mission and vision. We are committed to keeping you connected, to meet your needs as high school parents and work with you to keep our school strong. To learn more about the Women's Club, view the charter.

The Women's Club organizes various activities through the year such as:

Men's Club

The Resurrection Men’s Club is a formal organization whose primary goal is to provide a school-based group for the adult males who play a significant role in the life of a Resurrection student. Membership of the Men’s Club includes fathers, stepfathers, grandfathers, uncles, or other adult male members of the student’s extended family who participate at any of the club meetings or events.

Men’s Club serves several purposes for Resurrection College Prep High School, including, organizing school-wide fundraising, enhancing the school grounds, and holding informal get-togethers and events. Through the Resurrection Men’s Club activities, the adult men in the lives of our students can also become better acquainted with their student’s teachers, school administrators, and friends.

Athletic Booster Club

Resurrection’s Athletic Booster Club sponsors activities and events to benefit our Athletic Program and to promote the exceptional athletic achievements of our young women. Parents can volunteer for a variety of activities and events throughout the school year and those opportunities will be communicated each month at the meeting and in the bi-weekly parent email from RES.
